I give permission for my Son/Daughter to participate with HMBACCE onsite activities. I understand that HMBACCE will provide transportation for any offsite activity and my child/ren's well-being is a priority. My signature releases and holds harmless HMBACCE and its Pastors, Volunteers, Leaders, Churches, Staff and other entities or persons directly or indirectly affiliated.
